FETO-MATERNAL OUTCOMES IN PATIENTS OF PLACENTA PREVIA WITH OR WITHOUT UTERINE SCAR AT GMMMCH SUKKER

Abstract

Objective: To assess the frequency of uterine scar in females with Placenta Previa and to compare the feto-maternal outcomes with or without uterine scar in patients of placenta previa


Methodology


Study Design: Descriptive Cross Sectional


Setting and duration: Department of Obstetrics and Gynaecology, Ghulam Muhammad Mahar Medical College Teaching Hospital from 01 January 2025 till 30 June 2025


Data Collection: Ethical clearance and informed consent were obtained and 100 eligible women were recruited. Structured proforma was used to obtain Demographic, Clinical, ultrasonographic, Maternal and Neonatal data. Participants were followed till the time of delivery and were followed up for haemorrhagic complications, blood transfusion, caesarean hysterectomy and admission to the intensive care unit. Gestational age, birth weight, Apgar scores and admission to a NICU were also documented. All the collected data was entered and analyzed on SPSS version 25


Results: Of the 100 women with placenta previa, 41 women (41.0%) had a uterine scar. Maternal complications were significantly higher in women with a scar: antepartum haemorrhage (70.7% vs. 40.7%, p=0.003), postpartum haemorrhage (73.2% vs. 28.8%, p<0.001), caesarean hysterectomy (41.5% vs. 10.2%, p<0.001), massive blood transfusion (46.3% vs. 13.6%, p<0.001), and ICU admission (61.0% vs. 11.9%, p<0.001). There were no significant differences with respect to neonatal outcome (p>0.05).


Conclusion: This study concluded that 41% of women with placenta previa had a uterine scar. The presence of a uterine scar was significantly associated with increased maternal morbidity, including antepartum and postpartum hemorrhage, caesarean hysterectomy, massive blood transfusion, and ICU admission
